Students planning to take a clinical or practicum course(s) Spring 2009 must submit completed preceptor forms by November 24, 2008. This does not apply to students taking
clinical courses through UOM.

Preceptor forms should be forwarded to Lori Stephens by fax at (615) 366-3953, by email at lori.stephens@tbr.edu or by mail to 1415 Murfreesboro Pike, Suite 350, Nashville, TN 37217. Students are required to keep a copy of all forms for their records and may be requested to upload forms into their course at the beginning of each semester.

Concentration Coordinators are responsible for approving clinical sites and preceptors. For questions related to approval of sites, please contact your respective coordinator.

For easy reference, please view the list of Pre-Approved Clinical Sites (pdf). These participating locations are already contractually approved by the Tennessee Board of Regents. However, you are not required to use a pre-approved site.
